To report a claim Monday through Friday during business hours, please call our office at 972-663-5190.

After Business Hours or on the Weekend

Please leave us a voicemail message or send an email and we will call you back during office hours. If you need immediate assistance, consult the list of provider companies. These carriers have emergency claims service and can take your information after business hours.
